Cotto began as a strong, aggressive fighter and changed his style as he went up in weight. In his early years, he represented Puerto Rico in international events as an amateur, competing in lightweight and light welterweight divisions. In the year 2004, Cotto won the WBO light welterweight title and defended it six times before moving up in weight. In 2006, he became the WBA welterweight champion by defeating Carlos Quintana but later suffered his first career loss to Antonio Margarito in 2008.

In the year 2010, Cotto moved up to light middleweight, winning the WBA title. He achieved Super champion status and won a rematch against Margarito in 2011. Cotto faced a highly anticipated match against Floyd Mayweather Jr. in 2012, losing the WBA (Super) title.

In the year 2014, Cotto won the unified WBC, Ring, and lineal middleweight titles by defeating Sergio Martínez, making him the first four-weight world champion from Puerto Rico. He defended the titles in 2015 but lost to Canelo Álvarez. After a year of inactivity, Cotto returned in 2017 to become the WBO light middleweight champion but lost the title in his final fight to Sadam Ali.
